Vermont Medical Marijuana Registry Application
Grassroots Vermont is a medical-only facility. All patients must be registered with the Medical Marijuana Registry, a division of The Vermont Department of Public Safety. Vermont Dispensaries do not accept out-of-state medical patients at this time.
Step 1: See if You Qualify
The following conditions and symptoms may qualify a patient for the use of medical marijuana: Cancer, Multiple Sclerosis, Human Immunodeficiency Virus (HIV), Acquired Immune Deficiency Syndrome (AIDS), Cachexia or wasting syndrome, Severe nausea, Chronic pain, Seizures, Glaucoma, Crohn’s Disease, Post Traumatic Stress Disorder, Parkinson’s Disease
Step 2: Submit An Application
It is simple for qualified patients and their caregivers to submit an application. Download the forms below (you’ll need the Health Care Professional Verification Form for your healthcare provider). Fill out the form out completely, make sure to choose Grassroots Vermont as your dispensary, send a $50 check and a color electronic photo on CD or flash drive to:

Step 3: Visit Grassroots Vermont
Call ahead for an appointment. We are happy to meet with you to suggest the best products for your needs—as well as to answer any questions or concerns you may have. Just be sure to bring your VMR identification card and a government-issued photo ID (like a driver’s license) as required by Vermont law. We look forward to seeing you.
