Step 1:

 See if you qualify

The following conditions and symptoms may qualify a patient for the use of medical marijuana:
Cancer
Multiple Sclerosis
Human Immunodeficiency Virus (HIV)
Acquired Immune Deficiency Syndrome (AIDS)
Cachexia or wasting syndrome
Severe nausea
Chronic pain
Seizures
Glaucoma
Crohn’s Disease
Post Traumatic Stress Disorder
Parkinson’s Disease

Step 2:

 Submit an application

It is simple for qualified patients and their caregivers to submit an application. Just download the correct forms (you’ll need the Health Care Professional Verification Form for your healthcare provider), fill them out and have the patient form notarized. Then mail them in with a $50 check and a color electronic photo on CD or flash drive to:

STEP 3:

 Visit Grassroots Vermont

Call ahead for an appointment. Our experts are happy to meet with you to determine the best products for your needs—as well as to answer any questions or concerns you may have.  Just be sure to bring your VMR identification card and a government-issued photo ID  (like a driver’s license), as well as an empty, locked container to safely transport your product  as required by Vermont law.
